Abstract

A pilot system for a mammography tele-consultation in TaiChungVeterans General Hospital (TCVGH), Taiwan is proposed in thispaper. It is developed and designed based on a distributed systemand implemented in a top-down high speed network hierarchy withcommunication security. It is also integrated with specificdiagnostic tools and collaboration awareness tools currentlyavailable in TCVGH and linked with the external internet, TaiwanAcademic Network (TANet) that will be upgraded into the nextgeneration, TANet2 in the next five years. The distributed systemis designed using a JAVA-based remote method invoking system. Itallows the transport of images through a firewall combined withperipheral applications that include a Digital Imaging andCommunications in Medicine (DICOM) server for image transmission,Picture Archiving and Communication System (PACS), HospitalInformation System (HIS) and computer-aided diagnostic systems viaa Java native interface. The communication security is implementedusing ElGamals public-key cryptosystem for identifications ofauthorization. Finally the specific mammography diagnostic toolsand collaboration awareness tools demonstrated in the proposedsystem are developed in accordance with the Breast ImagingReporting And Data System (BI-RADS) suggested by the AmericanCollege of Radiology.
